My Planned Care

My Planned Care gives you advice and support while you wait and helps you to prepare for your hospital consultation, treatment, or surgery.

This includes giving you information about waiting times at your hospital and other supporting and local services while you wait.

  • If you are not sure which specialty you have been referred to, please check any letters that you have received from your hospital as this information should be included in these

Patient Triage

Ryeland Surgery offers the option of Patient Triage to assess routine problems. 

Online systems are not suitable for emergencies or the most urgent problems: in an emergency call 999, for an urgent problem call 111 or the surgery.


Why Patient Triage?

It is no secret that GP Practices face increasing pressure to help patients get the right medical advice they need, as quickly as possible.

At Ryeland Surgery we pride ourselves on continually looking at new ways to improve our service. Therefore, in order to ensure we meet the ever increasing demand and needs of our patients whilst ensuring resources are utilised appropriately, we have reviewed the way we deliver care and are looking at new ways of supporting our patients.

What is Patient Triage?

Patient Triage is an evidence based online platform that enables patients to request medical advice from their GP by simply completing an online consultation form. During the process patients may be provided with advice for self-help or to contact other services to receive the care that aligns with their symptoms. For example, the patient may be signposted to a pharmacy for minor ailments or emergency services for signs of critical illness. Patients can submit a Patient Traige for advice on any condition.

Following submission, you will receive contact from the practice within 2 working days with further advice or for an appointment to be arranged.

What are the benefits to patients?

Patients will:

  • Patients can send an online consult at a time convenient to them and not have to wait until the surgery is open
  • Receive medical advice 24/7 via online consult self help online services
  • Patients have their symptoms remotely assessed and be contacted within two working days potentially without the need to visit the surgery in person
  • Patients won’t necessarily have to take time off work to attend an appointment

Ryeland Surgery would like to invite you to use the NHS App

The simple and secure way to access a range of NHS services on your smartphone or tablet.

What the NHS App does

Use the NHS App to:

  • get advice about coronavirus – get information about coronavirus and find out what to do if you think you have it
  • order repeat prescriptions – see your available medicines, request a new repeat prescription and choose a pharmacy for your prescriptions to be sent to
  • book appointments – search for, book and cancel appointments at your GP surgery, and see details of your upcoming and past appointments
  • check your symptoms – search trusted NHS information and advice on hundreds of conditions and treatments, and get instant advice or medical help near you
  • view your medical record – securely access your GP medical record, to see information like test results, allergies and medications. Consultation entries and documents added after 04.10.23 (or since the date of registration if after 04.10.23) may also be available.
  • register your organ donation decision – choose to donate some or all of your organs and check your registered decision
  • find out how the NHS uses your data – choose if data from your health records is shared for research and planning

Other services in the NHS App

If your GP surgery or hospital offers other services in the NHS App, you may be able to:

  • message your GP surgery, doctor or health professional online
  • consult a GP or health professional through an online form and get a reply
  • access health services on behalf of someone you care for
  • view your hospital and other healthcare appointments
  • view useful links your doctor or health professional has shared with you

Keeping your data secure

After you download the app, you will need to set up an NHS login and prove who you are. The app then securely connects to information from your GP surgery.

If your device supports fingerprint detection or facial recognition, you can use it to log in to the NHS App each time, instead of using a password and security code.
